Below is an attached list of documents for GHVP packet submissions. Please refer to the latest version of the GHVP-0: Submission Checklist to confirm you are including the right documents based on the type of submission (new or renewal) and based on your client's situation.
The GHVP packet is now submitted digitally through an online Adobe form that will allow us to better track all submissions and ensure timely and accurate processing. Packets will first be reviewed by the Regional Field Office and then advance to the Central Office for a final review and processing.
