Are you or a loved one experiencing homelessness in Georgia? Do you have a behavioral health challenge?
Projects for Assistance in Transition from Homelessness (PATH) Teams conduct outreach to individuals who are experiencing homelessness and have behavioral health needs, including severe mental illness and substance use. PATH Teams provide case management to support connections to services and housing and can meet individuals wherever they are.
Available Resources from PATH Teams:
- Permanent housing
- SSI disability
- Free healthcare and dental
- Birth certificate, state ID, TB test
- Employment training and job placement resources
There are five PATH Teams in DBHDD Region 3 serving the Metro Atlanta Area and one team in each of DBHDD;s other five regions, for a total of ten teams with nine provider agencies.
