Attached is a fillable PDF version of the GHVP-17. A GHVP client or eligible household member may request a live-in aide if they have a need as verified by a licensed professional.
This form should be submitted with the initial client referral to GHVP so that the Notice to Proceed can provide for an additional bedroom for exclusive use by the Live-In Aide.
As governed by the Americans with Disabilities Act, in the event the need for a Live-In Aide is obviously evident, there is no need for certification, nor should it be requested.
A Live-In Aide must meet the following criteria:
- Must provide essential care to or for the tenant,
- Must not be obligated to support the tenant (i.e. spouse or dependent), and,
- Wouldn't otherwise live in the unit except to provide essential care for the tenant.
A Live-In Aide can be a family member but must meet the above criteria.
Live-In Aides will be included on the GHVP-2: Lease Addendum as an Occupant and they do not have tenancy rights, meaning if the individual they are providing care for moves out, they must move out as well. Their income should not be included in the rent and income calculations on the GHVP-5 form.
