The Need for Supportive Housing (NSH) Survey is an assessment tool used to determine client need and eligibility for supportive housing resources. Conducting the survey is the first step in the Unified Referral process and in the process to receive a potential referral to the Georgia Housing Voucher Program (GHVP). If the survey determines an individual is eligible, they can proceed to the referral stage where additional information and documentation will be required and can be submitted via the NSH portal.
Only provider agencies and provider staff authorized by DBHDD's Office of Supportive Housing (OSH) will receive access to the NSH application in order to conduct surveys. Conducting the assessment and utilizing the portal requires training in advance and can be provided by OSH. You can request training by submitting a ticket request.
If you are a DBHDD provider with agency access and you have an issue with access or need to request a login, you can use the help ticket form to request access.
