The GHVP-9: Notice to Stop Payment form is used to inform DBHDD that GHVP payments on a particular unit should be halted. This is for individuals who are staying in the program and is most often necessary when an individual is moving to a new property or the property ownership is changing or has changed and payments need to go to a new entity.
GHVP-9 is a new form created to more clearly distinguish between when individuals are undergoing a change in landlord vs. those who are leaving the program.
GHVP-9 is only to stop GHVP payments and not for requesting discharge from the program. Please use a GHVP-8 to request discharge.
After halting GHVP payments with this form, in order to re-start GHVP payments, a provider must complete a GHVP-4 with the new property payment information.
