Below is the contact information that Region 6 community based providers should utilize to reach out to the Housing Support Program providers serving Region 6 for care coordination, questions, etc.
The HSP provider is assigned to each individual by DBHDD as GHVP referrals are approved (your agency does not make direct referrals to the HSP) and will be identified on the Notice to Proceed you receive back from DBHDD.
Please reach out to the Primary Contact identified below for the assigned HSP provider. If you do not receive a response within one business day, please reach out to the secondary contact. If you have challenges reaching the HSP provider, please reach out to the DBHDD Regional Housing Coordinator.
