If you are having a challenge and haven't been able to receive support from your provider agency and/or case manager, you can reach out directly to DBHDD through our online support request form:
Select "GHVP Client" after entering your email and we will ask for some specific pieces of information to help get you assistance more quickly.
When contacting us for assistance, please provide the following information so that we can get it to the right team members:
- First and Last name
- Complete Address (including Apt #)
- Provider Agency and Case Manager (if applicable)
