This form is only used to initiate complete program discharge from GHVP. It also must be completed through the new online process outlined below.
If an individual is seeking new GHVP housing, they should not be discharged from the program and this form should not be used. To stop GHVP payments, instead use the GHVP-9.
Discharge Request Process
Please ensure you understand the process and requirements below before submitting a request.
GHVP-8 Online Form
1. Communication with Regional Field Office
Prior to a provider submitting a discharge request, a provider must discuss the situation with the Regional Field Office to confirm agreement with the request for discharge. If the Regional Field Office is initiating the discharge request, the provider should be informed by the RFO.
2. Submission of Request
Requests are not automatically approved - they will be reviewed by DBHDD and approval must be received before any discharge action can be taken by the provider.
Upon submitting the online Adobe Form, you will receive a verification email at the email provided on the form to ensure it was indeed you who submitted the form. Click the link to verify it was you and this will complete the process of submitting your request for review.
3. Request Review
The request will be reviewed by the applicable Regional Field Office for accuracy and complete information, including the expectation of a clear justification for discharge and any supporting documentation (if applicable). You will receive notice of approval or denial by email.
In Event of Request Denial
If the request is denied, a reason will be provided (e.g. insufficient information or the individual is not appropriate for discharge) and the provider can resubmit the request to provide any additional information that was missing. If the individual is identified as not appropriate for discharge, the provider must return to case conferencing the individual with the Regional Field Office.
4. Finalizing Discharge
When a request is approved, the staff person that submitted the request will receive an email notice. The Office of Supportive Housing team will then process the discharge to ensure the payment system is updated to stop payment and reflect that the individual has been formally discharged.
At this point the provider may initiate discharge from Housing Support services, as applicable.

Once discharged, in order to regain access to GHVP, an individual will have to go through the referral process again to be considered for a new voucher.
