Section 1: Questions 1 through 23 (Required)
- Question 1: Lease Type:
- Select from the drop-down option if this is a new lease or the renewal of an existing lease.
- Question 2: Region:
- Select the appropriate region from 1 through 6.
- Question 3: Name of Individual:
- Enter the full name of the individual as it is on the approved NTP.
- Question 4: NTP Number
- Enter the individual’s NTP number.
- Question 5: CID Number:
- Enter the individual’s CID number.
- Question 6: Provider Agency
- Enter the name of the provider agency completing this form
- Question 7: Provider Staff Contact Name
- Provide the name of the provider staff person submitting the form.
- Question 8: Provider staff contact Email
- Provide the provider agency staff person’s email address.
- Question 9: Provider Staff Contact Phone Number
- Provide a phone number for the provider staff person.
- Question 10: Address Location of Unit
- Provide the physical address of the intended unit.
- Question 11: County of Residence
- Identify the county for the intended unit.
- Question 12: Number of bedrooms
- Provide the number of bedrooms in the intended lease unit.
- Question 13: Number of household members
- Provide the number of people in the individual’s household.
- Question 14: Household Income
- Identify the household’s anticipated monthly income.
- Question 15: Lease Amount for the Unit
- Provide the lease amount for the unit intended for lease, exactly as it appears on the uploaded lease.
- Question 16: Does the lease amount meet the payment standard?
- Select the response from the dropdown options: “Yes” or “No”
- Question 17: Is the individual rental shared within the limits?
- Select the response from the dropdown options: “Yes” or “No”
- Question 18: Does the landlord agree to sign up for direct deposit?
- Select the response from the dropdown options: “Yes” or “No”
- Question 19: Does the landlord agree to the Housing Quality Standard (HQS) inspection?
- Select the response from the dropdown options: “Yes” or “No”
- Question 20: Does the landlord agree to make repairs as needed if determined by HQS inspection?
- Select the response from the dropdown options: “Yes” or “No”
- Question 21: Has the landlord submitted their W-9?
- Select the response from the dropdown options: “Yes” or “No”
- Question 22 - Has the landlord submitted IRS Form 147-C or their EIN Confirmation Letter (Form CP 575-A)?
- Select the response from the dropdown options: “Yes” or “No”
- Question 23 - Has the landlord submitted a social security card (if an individual owner)?
- Select the response from the dropdown options: “Yes” or “No”
- Select No if the property is a commercial property. Only the 147C is required in this case.
Section2: Comment Section
A comment text box is on the form to add any additional information that would be useful to help DBHDD make an informed decision
Section 3: Upload of Required Documents Section (Required)
In this section, providers are required to upload the following:
- GHVP-5 Form for the unit to be rented, indicating program standards are being met.
- A current W-9 Form from the property owner, and
- For commercial properties: IRS Form 147-C, OR, as an alternative, the business' EIN Confirmation Letter from the IRS (IRS Form CP 575-A)
- For properties without an associated business entity, you must submit a copy of the Landlord’s social security card to confirm their identity for payment.

Last Step: Attestation, Signature, and Email Confirmation (Required)
After completing all the required fields of the form and uploading all the required documents, read and acknowledge the attestation by digitally signing the form. Enter your email address to receive an email to confirm the user submitting this form.

Email Confirmation (Required)
When a message titled "Just One More Step" displays, check your email to complete the submission of the form.
Note: The web form is not fully submitted until you have validated your signature through the email link received.
In the email you receive, click “Confirm my email address”. Upon clicking this link, you will have successfully validated both your email address, your signature, and submitted the web form.
The next step is for DBHDD to review and make a decision. You will receive an email notifying you of the decision, either approval or denial.
- In the event of denial, an explanation should be included in the email.
- Approval means the inspection request process can move forward and the apartment is approved for leasing and move-in upon the passing of the inspection.
