DBHDD is an official partner with the Georgia Department of Labor (DOL) and we are now able to request wage statements from DOL as a part of the renewal certification process.
👉 This is a requirement for all RFTA submissions.
Please follow the process outlined below:
🔄 DOL Wage Statement Request & Recertification Process
- Start Early
Begin the recertification process at least 90 days before the lease expiration date. - Submit a DOL Wage Statement Request via Zendesk
- Go to the Supportive Housing Help Center
- Select “Submit a request” --- one request per individual
- Complete the form with the following:
- Your email address
- Who is submitting: Select DBHDD-Contracted Provider
- Housing Program Request Type: Select GHVP
- Type of Request: Select DOL Lookup
- DBHDD Region (1–6)
- CID Number
- SSN (to verify wage history)
- Reason for DOL Request: Select Housing Renewal Review
- Subject: Enter the client’s name
- Message: Brief explanation (e.g., “I am requesting DOL wage statement for annual recertification.”)
- Optional: Attach any relevant documents

- Verify Income
After receiving the DOL wage statement, compare it with any income reported since the last recertification. If the individual is no longer working at a specific notice – a separation notice should be requested and submitted. - Submit with GHV-5
Include the DOL wage statement with the GHV-5 form during the RFTA process to document wage verification.
đź“„ Additional Documentation Requirements
- If Employed:
Include 4 recent paycheck stubs. These are required in addition to the DOL wage statement and must be reviewed prior to the lease signing due to past discrepancies. - If Receiving Disability Benefits:
Include documentation with the RFTA. - If Individual Has Zero Income:
Submit a signed Zero Income Form with the RFTA. We also need to know why there is no income and what is the plan to gain income.

This process will help ensure more accurate income verification and reduce errors before leases are signed. Please make it a priority to stay on top of the paperwork and timeline to avoid delays in housing renewals.
